Community Aged Care
Home and community based ageing supports include personal care, respite, domestic assistance and social support.
Supports can be provided in-home and in the community based on individual needs and choices.
- Assistance with hygiene and self care
- Support to participate in the community
- Social contact and group activities
- Support to attend appointments
- Assistance with shopping and errands
- Support to complete household tasks

Care Finder Program
Care Finder is a free service that helps people to:
- Set up aged care services;
- Change aged care services;
- Connect with other supports like housing, health or mental health.
Nexus Primary Health provide the Care Finder program in the Mitchell, Murrindindi, Strathbogie councils.
